HEALTHCARE PROVIDER NETWORK

The Healthcare Provider Network for NHHP is provided by First Health – A Coventry Health Care Company. Click to search for an in-network provider or call 800-226-5116.

Foundation Medical Partners and Southern NH Medical Center are also available as an in-network providers for NHHP. Foundation Medical Partners includes over 195 providers serving the greater Nashua community.

COVERED BENEFITS

Benefits are available for medically necessary services and supplies such as inpatient medical and surgical services, outpatient medical and surgical services, diagnostic services, prescription drugs, and other covered services. All services must be prescribed by a physician and related to a covered injury or sickness. Benefits are subject to exceptions and limitations as stated in the policy. An overview of covered benefits are listed in the NHHP Brochure. For a complete listing of covered benefits, terms, and conditions, please refer to the specific Benefit Plan's Outline of Coverage.

CLAIM SUBMISSION

If your Healthcare Provider is in-network, they will take care of filing your claims for services rendered. If you need to submit a claim yourself, be sure the following information is included:

Name of Insured
Insured's Social Security Number
Name of Patient
Date(s) of Service
CPT (Current Procedural Terminology) Code
ICD-9 Code (Diagnosis)
Amount charged
In case of an accident, in addition to the above, please include:
Date
Time
Place
How the accident happened
Also, please note if it is work related.
